Lab Web Portal (LWP) News and Resources

As of 12/15/25 the ETOR portal for ordering COVID-19 testing is no longer available. To order COVID-19 testing, please fill out the Virology Submittal Form (DHHS-3431) and write “COVID-19 testing” under “other” in the “Infectious Agent(s) Suspected or Test(s) Requested” section at the bottom of the form.
